pKVM provides mutual distrust between host kernel and protected VMs(pVM)
One solution to provide DMA isolation in this model, is to move the IOMMU
control to the hypervisor and para-virtualize the IOMMU interface for
the host and guest kernel. (none of them have direct access to IOMMU
programming interface).
In the case of device assignement, the host can't map memory in for the
guest kernel (as it is not trusted).
So, what mainly needs to be done is to assign a blocking domain, when
VFIO assigns the device to user space, so it can't issue any DMA, and
when the guest take control it can program the IOMMU through hypervisor
with collapsed translation (IOVA->PA directly).

The vfio-platform SET_IRQS ioctl currently allows loopback triggering of
an interrupt before a signaling eventfd has been configured by the user,
which thereby allows a NULL pointer dereference.
Rather than register the IRQ relative to a valid trigger, register all
IRQs in a disabled state in the device open path. This allows mask
operations on the IRQ to nest within the overall enable state governed
by a valid eventfd signal. This decouples @masked, protected by the
@locked spinlock from @trigger, protected via the @igate mutex.
In doing so, it's guaranteed that changes to @trigger cannot race the
IRQ handlers because the IRQ handler is synchronously disabled before
modifying the trigger, and loopback triggering of the IRQ via ioctl is
safe due to serialization with trigger changes via igate.
For compatibility, request_irq() failures are maintained to be local to
the SET_IRQS ioctl rather than a fatal error in the open device path.
This allows, for example, a userspace driver with polling mode support
to continue to work regardless of moving the request_irq() call site.
This necessarily blocks all SET_IRQS access to the failed index.

The .remove() callback for a platform driver returns an int which makes
many driver authors wrongly assume it's possible to do error handling by
returning an error code. However the value returned is ignored (apart
from emitting a warning) and this typically results in resource leaks.
To improve here there is a quest to make the remove callback return
void. In the first step of this quest all drivers are converted to
.remove_new(), which already returns void. Eventually after all drivers
are converted, .remove_new() will be renamed to .remove().
Trivially convert this driver from always returning zero in the remove
callback to the void returning variant.

Like vfio-pci, there's also a base module here where vfio-amba depends on
vfio-platform, when really it only needs vfio-platform-base. Create a
sub-menu for platform drivers and a nested menu for reset drivers. Cleanup
Makefile to make use of new CONFIG_VFIO_PLATFORM_BASE for building the
shared modules and traversing reset modules.

Use GFP_KERNEL_ACCOUNT for userspace persistent allocations.
The GFP_KERNEL_ACCOUNT option lets the memory allocator know that this
is untrusted allocation triggered from userspace and should be a subject
of kmem accounting, and as such it is controlled by the cgroup
mechanism.

This creates the iommufd_device for the physical VFIO drivers. These are
all the drivers that are calling vfio_register_group_dev() and expect the
type1 code to setup a real iommu_domain against their parent struct
device.
The design gives the driver a choice in how it gets connected to iommufd
by providing bind_iommufd/unbind_iommufd/attach_ioas callbacks to
implement as required. The core code provides three default callbacks for
physical mode using a real iommu_domain. This is suitable for drivers
using vfio_register_group_dev()

Claim group dma ownership when an IOMMU group is set to a container,
and release the dma ownership once the iommu group is unset from the
container.
This change disallows some unsafe bridge drivers to bind to non-ACS
bridges while devices under them are assigned to user space. This is an
intentional enhancement and possibly breaks some existing
configurations. The recommendation to such an affected user would be
that the previously allowed host bridge driver was unsafe for this use
case and to continue to enable assignment of devices within that group,
the driver should be unbound from the bridge device or replaced with the
pci-stub driver.
For any bridge driver, we consider it unsafe if it satisfies any of the
following conditions:
1) The bridge driver uses DMA. Calling pci_set_master() or calling any
kernel DMA API (dma_map_*() and etc.) is an indicate that the
driver is doing DMA.
2) If the bridge driver uses MMIO, it should be tolerant to hostile
userspace also touching the same MMIO registers via P2P DMA
attacks.
If the bridge driver turns out to be a safe one, it could be used as
before by setting the driver's .driver_managed_dma field, just like what
we have done in the pcieport driver.

This is the standard kernel pattern, the ops associated with a struct get
the struct pointer in for typesafety. The expected design is to use
container_of to cleanly go from the subsystem level type to the driver
level type without having any type erasure in a void *.

pm_runtime_get_sync() will increment pm usage counter even it
failed. Forgetting to call pm_runtime_put will result in
reference leak in vfio_platform_open, so we should fix it.

Since commit 7723f4c5ec ("driver core: platform: Add an error
message to platform_get_irq*()"), platform_get_irq() calls dev_err()
on an error. As we enumerate all interrupts until platform_get_irq()
fails, we now systematically get a message such as:
"vfio-platform fff51000.ethernet: IRQ index 3 not found" which is
a false positive.
Let's use platform_get_irq_optional() instead.
