CATS sets its swizzle function to zero, which at the moment means
that no swizzling is required. Make this explicit for CATS.

Some platforms mark their hw_pci structure as __initdata, which means
it will be discarded after init time. Storing pointers to __initdata
in long lived data structures is a potential source of problems, and
in this case, sys->hw is unused apart from its initialization.
So, lets remove this member and its initializer.

The dl2k driver's rio_ioctl call has a few issues:
- No permissions checking
- Implements SIOCGMIIREG and SIOCGMIIREG using the SIOCDEVPRIVATE numbers
- Has a few ioctls that may have been used for debugging at one point
but have no place in the kernel proper.
This patch removes all but the MII ioctls, renumbers them to use the
standard ones, and adds the proper permission check for SIOCSMIIREG.
We can also get rid of the dl2k-specific struct mii_data in favor of
the generic struct mii_ioctl_data.
Since we have the phyid on hand, we can add the SIOCGMIIPHY ioctl too.
Most of the MII code for the driver could probably be converted to use
the generic MII library but I don't have a device to test the results.

Add nescesary mlme callbacks to satisfy "iz list" request from user space.
Due to 6lowpan device doesn't have its own phy, mlme implemented as a pipe
to a real phy to which 6lowpan is attached.

Fragmentation is usefull if multiple devices are connected to an E1
interface. Each fragment will have a subset of the available timeslots.
These devices require a cascde connection or a multiplexer.

commit a4910b7444 has broken promiscuous
mode, which is never set. port->promisc just reflects the last setting
of PROMISCUOUS mode to avoid doing an extra hypercall when it's already
set.
However, since it may fail because of hypervisor permissions, we should
still respect the multicast settings and not simply exit after setting
promiscuous mode.

There was a bug in the mask of regtype parameter for registering a
multicast filter. It was ignoring the scope bit, which was wrongly being
used for all filters. The SCOPE_ALL value adds a filter that allows all
multicast packets and ignores the MAC parameter, just what allmulticast
needs. The normals filters, however, should not use SCOPE_ALL.

This function is used for suspend of S1/S3/S4 and driver remove.
It sets MAC/PHY based on the WoL configuation to get lower power
consumption.
atl1c_phy_power_saving is renamed to atl1c_phy_to_ps_link, this
function is just make PHY enter a link/speed mode to eat less
power.
REG_MAC_CTRL register is refined as well.

Many magic data are re-configured for PHY during its reset operation
based on chip type to get better compability and stability.
REG_PHY_CTRL register may be configured by BIOS before enter OS.
so, the driver can't directly write to it without any Read-Op.
this change also affect suspend and phy_disable routines.
PHY debug ports and extension registers are refined as well.

bit 17/18 of reg1424 must be clear for l2cb 1.x, or it will cause
the write-reg operation fail without cable connected.
so, please do connect the cable when apply this patch to the driver
to make sure these 2bits are cleared by new driver.
The revised code is move to al1c_reset_mac.

when do reset MAC, speed/duplex control right should be transferred

phy register is read/write via MDIO control module ---
that module will be affected by the hibernate status,
to access phy regs in hib stutus, slow frequency clk must
be selected.
To access phy extension register, the MDIO related
registers are refined/updated, a _core function is
re-wroted for both regular PHY regs and extension regs.
existing PHY r/w function is revised based on the _core.
PHY extension registers will be used for the comming
patches.

MCC Response CQEs are processed as part of NAPI poll routine and
also synchronously. If MCC completions are consumed by NAPI poll
routine, wrong status is returned to synchronously waiting routine.
Fix this by getting status of MCC command from command response
instead of response CQEs.

EQ is getting armed wrongly in INTx mode as INTx interrupt is taking
some time to deassert. This can cause another interrupt while NAPI is
scheduled and scheduling a NAPI in interrupt does not take effect.
This causes interrupt to be missed and traffic stalls. Fixing this by
preventing wrong arming of EQ.

VLAN and multicast hardware filters are limited and can get
exhausted in adapters with many PCI functions. If setting
a VLAN or multicast filter fails due to lack of sufficient
hardware resources, these packets get dropped. Fix this by
switching to VLAN or multicast promiscous mode so that these
packets are not dropped.

- Convert the old uid mapping functions into compatibility wrappers
- Add a uid/gid mapping layer from user space uid and gids to kernel
internal uids and gids that is extent based for simplicty and speed.
* Working with number space after mapping uids/gids into their kernel
internal version adds only mapping complexity over what we have today,
leaving the kernel code easy to understand and test.
- Add proc files /proc/self/uid_map /proc/self/gid_map
These files display the mapping and allow a mapping to be added
if a mapping does not exist.
- Allow entering the user namespace without a uid or gid mapping.
Since we are starting with an existing user our uids and gids
still have global mappings so are still valid and useful they just don't
have local mappings. The requirement for things to work are global uid
and gid so it is odd but perfectly fine not to have a local uid
and gid mapping.
Not requiring global uid and gid mappings greatly simplifies
the logic of setting up the uid and gid mappings by allowing
the mappings to be set after the namespace is created which makes the
slight weirdness worth it.
- Make the mappings in the initial user namespace to the global
uid/gid space explicit. Today it is an identity mapping
but in the future we may want to twist this for debugging, similar
to what we do with jiffies.
- Document the memory ordering requirements of setting the uid and
gid mappings. We only allow the mappings to be set once
and there are no pointers involved so the requirments are
trivial but a little atypical.
Performance:
In this scheme for the permission checks the performance is expected to
stay the same as the actuall machine instructions should remain the same.
The worst case I could think of is ls -l on a large directory where
all of the stat results need to be translated with from kuids and
kgids to uids and gids. So I benchmarked that case on my laptop
with a dual core hyperthread Intel i5-2520M cpu with 3M of cpu cache.
My benchmark consisted of going to single user mode where nothing else
was running. On an ext4 filesystem opening 1,000,000 files and looping
through all of the files 1000 times and calling fstat on the
individuals files. This was to ensure I was benchmarking stat times
where the inodes were in the kernels cache, but the inode values were
not in the processors cache. My results:
v3.4-rc1: ~= 156ns (unmodified v3.4-rc1 with user namespace support disabled)
v3.4-rc1-userns-: ~= 155ns (v3.4-rc1 with my user namespace patches and user namespace support disabled)
v3.4-rc1-userns+: ~= 164ns (v3.4-rc1 with my user namespace patches and user namespace support enabled)
All of the configurations ran in roughly 120ns when I performed tests
that ran in the cpu cache.
So in summary the performance impact is:
1ns improvement in the worst case with user namespace support compiled out.
8ns aka 5% slowdown in the worst case with user namespace support compiled in.

- Transform userns->creator from a user_struct reference to a simple
kuid_t, kgid_t pair.
In cap_capable this allows the check to see if we are the creator of
a namespace to become the classic suser style euid permission check.
This allows us to remove the need for a struct cred in the mapping
functions and still be able to dispaly the user namespace creators
uid and gid as 0.
- Remove the now unnecessary delayed_work in free_user_ns.
All that is left for free_user_ns to do is to call kmem_cache_free
and put_user_ns. Those functions can be called in any context
so call them directly from free_user_ns removing the need for delayed work.

dsp_spl_jiffies need to be the same datatype as jiffies (which is ulong).
If not, on 64 bit systems it will fallback to schedule the DSP every jiffie
tic as soon jiffies become > 2^32.
